CONSENT FOR DELIVERY OF STATEMENTS BY ELECTRONIC MEDIA

(“Customer”) requests that MBF Clearing Corp. (“MBF”) provide customer account statements (monthly and daily statements) by e-mail. Customer warrants that electronic delivery is sufficient for Customer’s needs and Customer acknowledges that by consenting to such delivery, account statements will no longer be sent via regular mail.

Customer warrants and represents that Customer has taken all action necessary so that the below-referenced e-mail address(es) will receive and be able to promptly print out the relevant Customer statements in the form transmitted (i.e., removal of any blockers or spam detectors with respect to e-mails being received from MBF or its agent).

Customer understands and acknowledges that there is a risk of failure of any electronic transmission, and Customer agrees that Customer will hold MBF harmless for any electronic transmission failure regardless of the source or cause of the failure. If Customer fails to receive any confirmation statement, Customer will contact MBF promptly. Customer acknowledges that failure to receive any statement will not relieve Customer of Customer’s obligation under the Customer Agreement or the Customer’s responsibility for any activity in Customer’s account including but not limited to trades, positions and/or margin requirements.

Customer agrees to notify MBF promptly of any changes to the information provided above. Customer warrants that in the event that the account is held in the name of a legal entity, , the undersigned is duly authorized to execute this form and Customer has provided, under separate cover, appropriate documentation establishing that the person executing this confirmation possesses the requisite authority to bind the entity.

I hereby authorize MBF to provide Customer account statements for the accounts specified above to corresponding e-mail address(es) as noted.
